4CYTE Canine 在第 14 天的 OM 评分与第 28 天的所有三项指标中,均被证实不劣于卡洛芬。
纳入标准:年龄 > 6 个月,因跛行就诊,经体格检查、血液学 / 临床化学检查及影像学检查确诊为骨关节炎,体重范围调整为 10–50 kg(原方案为 13–50 kg,后因外部效度考虑修正);
排除标准:骨关节炎治疗前 14 天内使用过相关药物(二甲亚砜、透明质酸钠、硫酸软骨素、氨基葡萄糖等),或患有其他并发疾病。
4CYTE Canine:每日 1 次随晚餐口服,连续 28 天,剂量按体重调整(10–<13 kg:2 勺 / 天;13–20 kg:4 勺 / 天;>20–25 kg:6 勺 / 天;>25–30 kg:6 勺 / 天;>30–38 kg:8 勺 / 天;>38–50 kg:8 勺 / 天),每勺含约 0.3 g 活性成分 Epiitalis®。
卡洛芬:每日 2–4 mg/kg 口服,首日可给予最高 4 mg/kg 的负荷剂量,按体重调整(10–<13 kg:首日半片,后续半片 / 天;13–20 kg:首日 1 片,后续 1 片 / 天;>20–25 kg:首日 1 片 + 半片,后续 1 片 / 天;>25–30 kg:首日 2 片,后续 1.5 片 / 天;>30–38 kg:首日 1.5 片 + 1 片,后续 1.5 片 / 天;>38–50 kg:首日 2 片 + 1 片,后续 2 片 / 天),每片含卡洛芬 50 mg。
主要终点:第 28 天主人跛行评分(OL)较第 0 天的改善,评分采用 4 级有序量表(1 = 无 / 轻微跛行;2 = 可站立行走但明显跛行;3 = 可无辅助站立但行走严重跛行;4 = 几乎无法站立 / 行走);
次要终点:兽医跛行评分(VL)、主人活动能力评分(OM),采用与 OL 相同的 4 级量表;
额外评估:第 14 天、28 天的血液学 / 临床化学检查,第 0 天的关节影像学检查,以及任意时间点的改善情况(Imp_Any)。

不良事件:1 只卡洛芬组犬出现呕吐并住院治疗,1 只卡洛芬组犬出现便血,1 只 4CYTE Canine 组犬拒食(通过诱食解决),均未出现严重不良反应。
非劣效性验证:4CYTE Canine 在第 14 天的 OM 评分与第 28 天的 OL、VL、OM 三项指标中,均被证实不劣于阳性对照药物卡洛芬;
疗效持续性:4CYTE Canine 组的跛行改善效果在第 14 天至第 28 天期间持续存在,而卡洛芬组的改善效果未呈现持续性,提示 4CYTE Canine 的作用机制可能为抗炎并逐步积累疗效,与 Epiitalis 的软骨保护作用机制一致;
临床意义:4CYTE Canine 为骨关节炎患犬提供了一种非甾体抗炎药之外的安全选择,尤其适用于需长期治疗或对 NSAIDs 不耐受的犬只。
试验因外部因素提前终止,样本量未达初始设计目标;
采用主观有序评分量表,可能存在评估偏倚;
未设置安慰剂对照组,无法排除安慰剂效应。
